I authorise Cairns Regional Council to use Centrelink Confirmation eServices to perform a Centrelink/DVA enquiry of my Centrelink or Department of Veterans’ Affairs customer details and concession card status to enable the business to determine if I qualify for a concession, rebate or service. *

I authorise the Australian Government, Services Australia (the department) to provide the results of that enquiry to Cairns Regional Council. *

I understand that:

the department will disclose personal information to Cairns Regional Council including my name, address, concession card type and status to confirm my eligibility for the Pensioner Rate Subsidy/Concession.
this consent, once signed, remains valid while I am a customer of Cairns Regional Council unless I withdraw it by contacting Cairns Regional Council or the department.
I can get proof of my circumstances/details from the department and provide it to Cairns Regional Council so my eligibility for the Pensioner Rate Subsidy/Concession can be determined.

You must be the primary holder of a valid:

2. Queensland Department of Veteran Affairs Repatriation Health Card for all conditions (Gold Card- with Q Prefix)
Please note that a Health Care Card, Seniors Card, Seniors Business Discount Card, or DVA White Card, does not entitle the holder to receive a concession.
The applicant must be the registered owner or life tenant (either solely or jointly) of the property within the boundaries of Cairns Regional Council which is his/her principal place of residence. The State Government Subsidy/Council Pensioner Concession is not applicable on vacant or leasehold land, or rental properties.
The residential address recorded with Centrelink must verify as the address for which pension subsidy/concession is being claimed.
Upon proof of your eligibility the State Government Subsidy/Council Pensioner Concession will commence from the date the application is received by Council and will not be applied retrospectively. If an application is received after 31 May or 30 November, the State Government Subsidy/Council Pensioner Concession will commence from the start of the next rating period, 1 July or 1 January.
Any rate notice issued prior to the date the application is received, must be paid by the due date for payment (as indicated on such notice) to prevent interest charges being incurred.
If applying under life tenancy, the applicant is required to provide a copy of a valid Will or Supreme or Family Court Order, showing they have the right to reside at the property and are responsible for the payment of rates for the property.
If the property is jointly owned and the other party is no longer responsible for the payment of rates and charges (i.e.divorced/separated), a Statutory Declaration will be required to accompany this application form to ensure the correct percentage of concession is given.
A new application form must be lodged with Council if you purchase another property, and it becomes your principal place of residence.
