Parent/Guardian Declaration

I acknowledge and agree to the following:

Supervision – Children must be accompanied by a parent or caregiver at all times.
What to bring – I will provide my child with snacks, sun-safe clothing, insect repellent, a hat, a drink bottle, and closed-in shoes.
Reservation confirmation – I understand that this reservation is not confirmed until I receive confirmation from the Program Supervisor.
Cancellations – If unable to attend, I will notify the Program Supervisor by 8:30am on the day of the program.
Illness – If my child is unwell, I will contact the Program Supervisor by 8:30am on the day to advise they will not be attending.
